As a first-wave Community-Based Organization (CBO) participating in New York State's 1115 Medicaid Waiver through HEALI, Mercy Haven helps Medicaid members address the social factors that impact their health and well-being.

Our team provides eligibility screenings, care coordination, and personalized navigation to connect individuals with the resources they need to achieve greater stability. Services include Housing Navigation, Housing Tenancy Supports, food security interventions, and referrals to community-based services that promote long-term health and independence.
