Supportive Housing

HEARTH: Healed, Enriched and Renewed Through Housing
Mercy Haven’s HEARTH program (Healed, Enriched, and Renewed Through Housing) provides independent living with built-in support services for 250 residents, including more than 60 children. With 115 housing units across Suffolk County, HEARTH offers stability and a fresh start to individuals and families—many previously living in shelters or on the streets.
Our housing specialists work closely with residents to develop individualized Support Plans that promote long-term success. These plans help individuals strengthen essential life skills such as budgeting, health management, and accessing community resources. Residents also receive education and vocational training support, legal advocacy, and wellness planning, empowering them to build a brighter, more independent future.
Supportive housing is a cost-effective, solution-driven approach to ending chronic homelessness, providing not just a place to live but the tools and resources needed for lasting self-sufficiency. Learn more about the benefits of supportive housing here.
