LIFE SKILLS COORDINATOR

Qualifications:

  • Bachelor’s Degree (Masters preferred in a behavioral health field or Vocational Rehabilitation Counseling)
  • Experience working with volunteers and people struggling with substance abuse and mental illness
  • Organizational and time management skills
  • Community relations, vocational/educational skill development
  • Clean Driver’s license
  • Monday-Friday w/flexibility for occasional evening and/or weekend hours

Specific Requirements:

  • Employee must observe the policies, procedures and rules of the agency.  A complete summary of these will be provided during the orientation phase.
  • Practice proper habits of hygiene and professional appearance.
  • Attend scheduled staff and departmental meetings.
  • Review and understand the Mission, Vision and Goals of the organization.

Responsibilities:

  • Overall development, management and oversight of all programs and life skills activities;
  • Supervision of residents and their responsibilities at ME and during REAP activities;
  • Review all contracts and fees with supervisor;
  • Supervision and implementation of all policies/procedures;
  • Outreach, creation of ongoing maintenance of new partnerships of support and MH communities;
  • Liaison with local providers or services to evaluate need and prevent duplication of services;
  • Develop and implement specialized employment skills training and career opportunities for MH participants and others needing this service;
  • Coordination of community garden;
  • Work with Agency staff to submit grants for any expansion;
  • Creation and ongoing maintenance of new partnerships of support;
  • Recruitment and supervision of needed volunteers to implement life skills programs;
  • Communicate and articulate with both residents and staff of on-going and changing needs for ADL, educational, job training, employment and social interaction opportunities;
  • Encourage participation of residents in all life skills programs;
  • Coordinate with supervisors and staff with program accessibility and participation;
  • To facilitate and make available to the REAP calendar existing community resources and programs as such information becomes available to REAP;
  • Supervise provision and maintain an inventory of program and stock supplies, i.e. educational materials, art supplies, pantry and food supplies, monitor petty cash;
  • Coordination, with staff, and provide transportation of  residents, as needed, to various life skill events;
  • Ability to maintain attendance records;
  • Proper preparation of workshops and set up and cleanup of workshop areas;
  • Responsible for other related activities.
  • Coordinate staff contacts with local service providers as needed to assist residents in accessing services and to jointly plan for discharge;
  • Work with support services on behalf of the residents’ best interest;
  • Assist and or model the various techniques required to maintain a clean and healthy living environment;
  • Responsible for other related activities.

COMMUNITY RESIDENCE ASSISTANT SUPERVISOR – F/T

Work in our Bay Shore Community Residence assisting the administrator with household management activities as well as assisting residents living with mental illness in the activities for daily living. Qualifications:

  • Bachelor’s Degree in human services or
  • Associate’s Degree in human services and three years’ experience in human services or
  • High school diploma and five years’ experience in human services
  • One year supervisory experience working with people with psychiatric disabilities
  • Strong organizational and managerial experience
  • Experience in the mental health field
  • Clean driver’s license
  • First Aid and CPR Certified

Responsibilities:

  • Supervise household management activities to ensure conformance with program standards,
  • regulations, and budgetary guidelines;
  • Scheduling of monthly fire drills;
  • Responsible for coordinating shopping, cooking, laundry, maintenance, recreation, meal planning and household functions, etc., of the household;
  • Coordinate staff contacts with local service providers as needed to assist residents in accessing services and to jointly plan for discharge;
  • Schedule staff coverage of residence along with administrator;
  • Convene staff for appropriate staff meetings when needed;
  • Maintenance and submission of payroll and petty cash requests;
  • Conduct scheduled house meetings with residents;
  • Ensure that residents are fully apprised of requirements of the program and ensure compliance;
  • Train and assist residents in activities of daily living skills and assist residents in financial management when necessary;
  • Provide counseling, behavioral intervention and crisis management service;
  • Responsible for on-call duties and proper notification of Administrator or Program Director of all medical/psychiatric emergencies; on-call duties of 3 residences on a rotating basis;
  • Attend inter and intra agency meetings as requested by administration;
  • Write progress notes on a daily basis of resident’s activities;
  • Responsible for medication procedures of residence;
  • Responsible for maintenance and cleanliness of staff office;
  • Responsible for the overall supervision of service planning, case conferences, discharge planning as well as Medicaid billing and rehabilitative services;
  • Assist and or model the various techniques required to maintain a clean and healthy living environment;
  • Responsible for other related activities.

COUNSELOR - FULL TIME (2 Positions)

ABOVE REQUIREMENTS ARE PREFERRED, however, High School Diploma

with 2-4 years’ experience in human services will be considered.

  • Clean Driver’s license
  • CPR & F/A Certificate (required within first three months of employment)
  • Work 4-days/wk on – 3 days/wk off

Responsibilities:

  • Participate in the planning/evaluation of individual service plans and monitor plan objectives;
  • Hands on training and assisting residents in the activities for daily living and documenting interaction;
  • Responsible for accompanying residents on recreational outings and travel training exercises;
  • Provide counseling, behavioral intervention and crisis management;
  • Assist in the day-to-day management of the community residence;
  • Responsible for administration of residence in the absence of the Assistant Administrator;
  • Notify the Assistant Administrator/Residential Administrator of any medical/psychiatric emergency;
  • Provide transportation as needed for shopping, medical appointments, etc;
  • Write progress notes on residents’ activities on a daily basis;
  • Monitors management of medication;
  • Involved with cleanliness of residence; Responsible for maintenance and cleanliness of staff office;
  • Advocate for residents’ benefits;
  • Teach residents skills needed to gain independence
  • Coordinate staff contacts with local service providers as needed to assist residents in accessing services and to jointly plan for discharge;
  • Work with support services on behalf of the residents’ best interest;
  • Assist and or model the various techniques required to maintain a clean and healthy living environment;
  • Responsible for other related activities
