What is expected of a HEARTH participant?
- Cooperation in applying for all necessary entitlements;
- Development of necessary linkages within the community to access needed
support services;
- Working with co-residents in creating a mutually acceptable living
experience;
- Responsible upkeep of the apartment/house;
- Seeking assistance from staff when additional help is needed;
- Independence in areas of medication management and Activities of Daily
Living (ADL), including budgeting.
What can a HEARTH participant expect?
- A unique living experience which provides;
- Safe, affordable and comfortable housing;
- Enhanced by staff assistance;
- Access to appropriate community services and entitlements;
- Remediation and enhancement of Activities of Daily Living (ADL), if needed;
- Management of entitlements, with legal support, if needed;
- Development of self-advocacy and empowerment skills;
- Furniture and supplies for household start-up;
- Resident Empowerment and Achievement Program (R.E.A.P.);
- Opportunities in our Supportive Employment Program.
Who can apply?
- Individuals who are living with mental illness;
- (Axis I diagnosis), or individuals who are living with mental illness and are HIV+ or Families/couples
living with a mental illness, or Families with an adult member living with mental illness who is HIV+;
- Individuals who are abstinent from drugs and alcohol;
